Below are the forms required for all student-athletes to be completed and returned by mail to the Department of Athletics. Adobe Acrobat Reader is required to view these forms. If you do not have Acrobat, it is available for download at www.adobe.com.

All first-year student-athletes must register at the NCAA Eligibility Center to be cleared for initial eligibility. Please remember to contact your school to forward your final grades and transcript to the Eligibility Center upon graduation. To determine the status of documents sent or to check on your eligibility status, contact the NCAA at (877) 262-1492.
